Personne n'est revenu sur l'enregistrement. Il y a quelque chose de pas clair et je ne vois pas comment ça s'explique.

... un évènement qui se serait passé moins d'une heure et demi auparavant. Il ne fonctionnait pas s'il est bien de 2h, par manque d'alimentation ou HS.
...A moins que des cables d'alimentation du CVR soient coupés au niveau du trou, pas de batterie tampon??

They have an explosive decompresion and it takes them over 2 hours to get that thing on the ground?!

Abstract::
On 25 July 2008, at 0922 local time, a Boeing Company 747-438 aircraft (registered VH-OJK) with 365 persons on board, departed Hong Kong International airport on a scheduled passenger transport flight to Melbourne, Australia. Approximately 55 minutes into the flight, while the aircraft was cruising at 29,000 ft (FL290), a loud bang was heard by passengers and crew, followed by the rapid depressurisation of the cabin. Oxygen masks dropped from the overhead compartments shortly afterward, and it was reported that most passengers and crew commenced using the masks. After donning their own oxygen masks, the flight crew carried out the 'cabin altitude non-normal' checklist items and commenced a descent to a lower altitude, where supplemental breathing oxygen would no longer be required. A MAYDAY distress radio call was made on the regional air traffic control frequency. After levelling the aircraft at 10,000 ft, the flight crew diverted to Ninoy Aquino International Airport, Manila, where an uneventful visual approach and landing was made. The aircraft was stopped on the runway for an external inspection, before being towed to the terminal for passenger disembarkation.
Subsequent inspection of the aircraft by the operator's personnel and ATSB investigators, revealed an inverted T-shaped rupture in the lower right side of the fuselage, immediately beneath the wing leading edge-to-fuselage transition fairing (which had been lost during the event). Items of wrapped cargo were observed partially protruding from the rupture, which extended for approximately 2 metres along the length of the aircraft and 1.5 metres vertically.
After clearing the baggage and cargo from the forward aircraft hold, it was evident that one passenger oxygen cylinder (number-4 from a bank of seven cylinders along the right side of the cargo hold) had sustained a sudden failure and forceful discharge of its pressurised contents into the aircraft hold, rupturing the fuselage in the vicinity of the wing-fuselage leading edge fairing. The cylinder had been propelled upward by the force of the discharge, puncturing the cabin floor and entering the cabin adjacent to the second main cabin door. The cylinder had subsequently impacted the door frame, door handle and overhead panelling, before falling to the cabin floor and exiting the aircraft through the ruptured fuselage.

Essayez de traduire un ch'tiot peu s'il vous plait.. L'idée de base de CRASH est de présenter l'information à des lecteurs francophones intéressés par les incidents et accidents aériens et qui ne maitriseraient pas suffisamment l'anglais pour satisfaire leur curiosité.

En regardant le rapport de l'ATSB, on voit que le cylindre nous a fait le coup de la balle de JFK.
Ma question :
comment un cylindre avec un gas compressé stocké verticalement peut se balader comme cela dans un avion? Si il y a eu rupture de la valve , le cylindre aurait du se projeter vers le bas et non vers le haut,non?

aïe, OK master, (pardon, oui), le mien est approximatif ... un résumé alors:
25/07 à 0217Zet 475 km NW Manille, bang après 55 min de vol, firsdt officer/co-pilote en charge, pilot auto s'est (?) disconnecté avec dépressurisation, Des messages ECAM sur altitude et statut de l'issue R2 s'affichaient(?). 2° officier se reposait zone crew et a rejoint le poste.
Mayday+descente en sécurité en 7 minutes après Idle, ralenti et aero freins, constat que 3 ILS (sur 3), les VHS/VOR, FMC? anormaux ou HS aterrissage manuel en visuel plus aide Radar de l'aeroport à Nino Aquino, avec full reverse et freins minimaux.
Préféré tirer l'appareil de la piste 06B au terminal où on a desembarqué les passagers.
Trou à l'emplanture aile 2X1,5m, des petits morceaux de materiaux (dont nid d'abeille) ont été trouvés sur le pylone du réacteur 3, face la zone explosée, une légère indentation et coupure trouvée sur le réacteur 3 proche du plan de rotation de la turbine, sur un panneau d'isolation acoustique ((acoustic lining-trad défaillante...)) pas de trace d'ingestion moteur. nombreux fils conducteurs cables sectionnés( >80), dont cable contrôle d'aileron (?)

Le CVR a enregistré 30min,41s après la dépressurisation (par comparaison au FDR), dont 24 min de vol au niveau 100 et le reste quelques min au sol.
Le (D)FDR est complet, son l'analyse se poursuit (cf attachement C).
QAR système::'QAR entered ‘sleep’ mode while the aircraft was in
cruise at FL290. The depressurisation event occurred 4 minutes and 48 seconds
later. Four seconds after the event, the QAR resumed recording data'.
Entré en mode veille au niveau 290, dépressurisation 4 minutes 48 s plus tard, 4 s après le QAR a 'sauve(garde'?) l'enregistrement des données. Suit la séquence détaillée.

L'oxygène passagers est constitué de 13 bouteille 1850 PSI en ACIER ((!!!)) chacune avec une 'valve?' intégrale de fermeture, un indicateur de pression et un système de protection contre sur-pression (disque frangible?)
Il y a une rangée de 7 cylindres dans la zone accidentée (les 6 autres étant situés au plafond de la soute vers le milieu de l'appareil)
constatations sur le 4° cylindre MANQUANT et explosé d'un banc de 7, propulsé vers le haut à travers la cabine et son plancher . Le cylindre N° adjacent a été Les investigations continuent pour comprendre pourquoi.

Porte R2 (N° 2 de la cabine à droite) voir Fig 12 dommages au cadre de la pote et trace de peinture VERTE (des cylindres), et poignée déplacée vers l'ouverture (de 120° sur les 180°).

Cylindres Info
All passenger oxygen cylinders installed in VH-OJK were of a single piece, heat- treated alloy steel construction. The missing (presumed failed) oxygen cylinder, part number 801307-0012, serial number 535657, was one of a batch of 94 cylinders manufactured in February 1996 to the DOT13 3HT1850 specification. The cylinders measured 22.8 cm outside diameter by 75.1 cm long (8.98 inches x 29.56 inches) and had a minimum 2.87 mm (0.113 inch) wall thickness.
Cylindres numérotés depuis le 'front of the cargo hold'
Resumé faisait partie un lot de 94 cylindres monocoques en alliage d'ACIER datés Février 1994. Pour chaque cylindre il y a TROIS lignes (tuyaux) acier connectées (càd: sécurité/surpression, alimentation et remplissage). Les deux dernières via un T sur une connection commune, avec regulateur de pression et transducteur integral (?)::

Les 3 lignes reliées à la bouteille manquante sont sectionnées (fractured)
Pas de trace de chauffage combustion.

Défaillance du cylindre:
investigations lancées avec le fabricant, recherche des icylindres de lasérie...: 'As the failed cylinder was not recovered, the ATSB is currently working with the aircraft manufacturer, other aircraft operators and the oxygen cylinder manufacturer, to obtain samples of cylinders from the same manufacturing batch as the failed item, to facilitate the ongoing investigation of all relevant issues'.

(Perso::il y a combien de trous 1 ou 2 dans le plancher cabine?, c'est hallucinant une trajectoire type fusée +ET Haspirée à ce point vers le trou de dépressurisation)

En relisant le rapport, le cylindre a perdu sa valve en heurtant la porte.
La, il s'est conduit comme une baudruche dont on laisse l'air s'échapper. Et il est repassé par le trou d'ou il venait. :shock:
Question:
Qu'est ce qui a fait que le cylindre s'est retrouvé propulsé vers le haut?
On sait qu'il n'ont pas trouvé de traces d'explosifs, mais ont ils verifié le frêt? Un récipient sous pression qui aurait explosé?

Willy, je comprends (supputation) plutôt que le cylindre se serait conduit SEUL comme une fusée DèS fissuration/fusée sans doute en partie basse (et latérale extérieure ET perçant avec d'autres débris vers l'extérieur en même temps) en s'arrachant de ses liaisons (impulsion brutale supplémentaire) dès qu'il s'est libéré des attaches de son rack support, en perçant le plancher cabine, le reste c'est plutôt des REBONDS en cercle dans la cabine contre les obstacles (genre billard), la valve (à l'opposé, et je crois bien abandonnée/scellée dans un des rebonds au plafond) ne doit pas avoir joué beaucoup sauf pour effet poinçonnant à travers le plancher. La bouteille n'avait alors (peut-être) plus beaucoup de moteur fusée en poussée après, l'impulsion suffit.
1850 Psi X %section ça doit faire quelques Tonnes. SOS les unités...
Impressionnant circuit peut être favorisé par la présence de matériaux plastiques plus souples jouant les bandes élastiques du billard.
PS:action ET réaction, si on découvre qu'un ?culot inférieur (selon mode de fabrication?) de la bouteille s'est séparé il aurait préparé le trou extérieur allant jusqu'à marquer (indentation) le réacteur 3 avec aspiration immédiate (et pas à la fin) quid d'un deuxième trou de passage?
Ils ont eu chaud, avion compris, balayer tout sur son passage sans exploser (ou enfoncer) la porte ou une commande vitale et dans une zone où personne n'a été gravement touché tient du grand coup de pot.
On n'a pas fini dans les compagnies de rechercher la provenancedes cylindres ou pièces connexes, d'aller regarder tout cela de près et de s'expliquer sur l'installation dans lesracks de beaucoup d'avions et pas que des Boeings à mon avis. Merci ATSB pour la rapidité de diffusion.

Je ne traduis pas tout ( :wink: ), on y apprend une seule nouvelle information :
Toujours pas de write-off pour Qantas ... Ils vont réparer l'avion !

Pour un coût de 10M dollars (australiens) soit 6M d'euros quand même. Il devrait être de retour en ligne en novembre 2008.

Sydney, 29 August 2008Qantas welcomed the release today by the Australian Transport Safety Bureau (ATSB) of its preliminary investigation report into the depressurisation incident and diversion to Manila of the B747-400 operating QF30 on 25 July 2008.
The report indicates that one of the aircraft's 13 passenger oxygen system cylinders failed and that while the fuselage and some flight systems sustained damage, the aircraft continued to operate safely and landed in Manila without further incident.
The Chief Executive Officer of Qantas, Mr Geoff Dixon, said the preliminary report was a factual account of the incident and investigation to date.
\"Our own investigations agree with the ATSB's preliminary conclusions,\" Mr Dixon said.
\"We will continue to assist the ATSB to ensure that factors that may have contributed to the incident are understood and that any corrective actions ultimately identified are implemented.\"
Mr Dixon said Qantas had completed an inspection of the oxygen systems across its B747-400 fleet on 1 August, which confirmed there were no safety issues.
He also said the B747-400 involved in the Manila accident was repairable at a cost of less than $10 million and would be back in service in November 2008.
\"Preparations for these repairs are underway, with the work to be carried out in Manila by Lufthansa Technik in consultation with Boeing and Qantas engineers.\"
Mr Dixon said despite the seriousness of the QF30 incident, Qantas' remained proud of its excellent safety reputation.

Toujours pas de write-off pour Qantas ... Ils vont réparer l'avion !

Pour un coût de 10M dollars (australiens) soit 6M d'euros quand même. Il devrait être de retour en ligne en novembre 2008.
